A paper certification program to keep schools heart-safe forced coordinators to visit in person. We rebuilt it on Formstack and Salesforce with e-signatures and automated tracking.

Replaced paper certification with instant digital submissions
A children's community hospital ran a paper-based certification program built to keep local schools heart-safe. Paper forms caused delays and had to be manually re-keyed into Salesforce. Multiple forms bred data-accuracy problems, and the process required two program coordinators to personally visit schools — an unsustainable workload as the program grew.
We built a platform on Formstack integrated with Salesforce, replacing paper with accessible, user-friendly web forms. It added e-signing for consent capture, full Salesforce integration for instant data access, automated expiration alerts for recertification, and configured reporting that flags expiring certifications and triggers automatic follow-up workflows.
